SUBMITTING THIS FORM CREATES A SECOND WEB FORM, WHICH YOU MUST PRINT, COMPLETE BY HAND, AND EITHER FAX OR AIRMAIL. THUS, MAKE SURE THAT YOU FILL OUT THIS FORM ON A COMPUTER FROM WHICH YOU CAN PRINT A WEB PAGE.
DIRECTIONS:

If you wish to give a talk or a poster, fill out and email the separate abstract form (but make sure you have included the title on this form). Include the registration fee even if you are asking for financial assistance.
2. Click on the "Submit and create Summary&Payment page" button (near the bottom) to email info to conference organizers and produce a Summary&Payment web page form.
3. Make a hard copy of the Summary&Payment form and print or type your credit card information (VISA or MC only), or attach a check or money order.

Registrants after Aug 1, 2002 are not guaranteed admittance, as attendance is being restricted and is near the limit.

Cancellations will be fully refunded only if received prior to October 1, 2002.
